King of Prussia, Pa.-based Universal Health Services ramped up AI investment in 2025 by scaling solutions across its revenue cycle operations as part of a broader enterprisewide AI strategy to drive efficiency and improve patient care.
“During 2025, we focused heavily on scaling solutions that reduce the burden of routine administrative tasks,” Marc Miller, CEO of UHS said during the system’s first-quarter earnings call. “We deployed and scaled a total of eight different use cases of AI solutions into our revenue cycle operations that are now yielding significant benefit on a going-forward basis.”
UHS saw operating income of $502.9 million (11.2% operating margin) in the first quarter of 2026, up from $454.8 million (11.1% margin) in the first quarter of 2025. The system’s net revenue was $4.5 billion and total operating expenses were $4 billion.
Mr. Miller said the system’s AI push is concentrated on two domains: operational to improve quality and patient experience, and administrative domain to increase efficiency.
In 2026, the system is turning that focus toward clinical operations.
“Included in our 2026 roadmap are several new use cases being designed and built with Hippocratic AI, which is one of our key AI solution partners,” Mr. Miller said. “It is too early to project the longer term financial impact of the 2026 initiative, although we expect them to be incremental to margins over time, and just as importantly, we expect them to have a real impact on quality and patient experience.”
Mr. Miller said these investments highlight how UHS’ efficiency is improving, from the financial side to improvements in denial management and revenue capture.
“We’re also doing a lot of things that are touching the patient,” he said. “Anything that we can do in those two areas is kind of where we’re going to focus for now. We’re not doing much in the clinical space yet, but I do think down the line we’ll see something there, as well.”
UHS comprises 29 inpatient acute care hospitals, 346 inpatient behavioral health facilities, 168 outpatient facilities and ambulatory care access points, an insurance offering and a physician network in 40 states, the District of Columbia, the United Kingdom and Puerto Rico. The system has approximately 101,500 employees.
