'Zevra: From Prodrugs to a Rare Disease Powerhouse'

Zevra Therapeutics wasn’t always a rare disease contender. Originally known as KemPharm, the company spent years focused on prodrug technology for ADHD and pain management. Recognizing that low-margin, high-volume primary care markets were a grueling battleground, management executed a bold strategic pivot: transforming into a specialized, high-margin rare disease commercial pharma.

Rebranded as Zevra Therapeutics, the company set its sights on ultra-rare disorders with massive unmet needs. Through the strategic acquisition of Acer Therapeutics and relentless execution on its flagship asset, MIPLYFFA (arimoclomol), Zevra achieved a monumental milestone in September 2024: receiving FDA approval for MIPLYFFA as the first-ever treatment for Niemann-Pick disease Type C (NPC)—a devastating, fatal neurodegenerative condition often referred to as "childhood Alzheimer's."

2. The $100 Million PRV Windfall: De-Risking the Balance Sheet

Upon FDA approval of MIPLYFFA in September 2024, Zevra was awarded a coveted Rare Pediatric Disease Priority Review Voucher (PRV).

* The Transaction: In October 2024, Zevra officially announced the sale of its PRV to an undisclosed buyer for $100 million in upfront cash.

* Why This Matters: PRV sales are 100% non-dilutive capital. Overnight, Zevra injected $100 million into its treasury without issuing a single new share or diluting existing equity holders, instantly setting it apart from typical micro-cap biotechs that rely on toxic equity offerings to stay afloat.

3. Financial Reality Check: Revenue, Net Income & Cash Runway

| Commercial Revenue | Ramping Rapidly. Revenue is driven by the commercial rollout of MIPLYFFA in NPC (commanding orphan drug pricing of $300k–$500k+ per patient annually) alongside OLPRUVA for Urea Cycle Disorders. Commercial momentum is accelerating as patient identification and insurance coverage expand. |

| Net Income / Profitability | Near Break-Even / Pivoting to Profitability. While GAAP net income has shown temporary net losses due to initial commercial launch investments and ongoing R&D for KP1077 (Idiopathic Hypersomnia), top-line growth and high gross margins are narrowing net losses fast toward sustainable profitability. |

| Cash Burn & Runway | Zero Near-Term Dilution Panic. Bolstered by the $100M PRV windfall plus commercial cash receipts, Zevra holds a formidable cash cushion extending its operational runway well into 2026/2027+. |

4. Key Investment Risks

* Commercial Execution & Patient Onboarding: NPC is an ultra-rare disease (~300–400 diagnosed patients in the US). Commercial success hinges on physician education, genetic screening, and navigating complex specialty pharmacy reimbursement.

* Clinical Trial Expense Risk: Ongoing development of KP1077 for Idiopathic Hypersomnia requires ongoing R&D spend, which could delay net income profitability if clinical costs expand.

* Legacy Debt Obligations: Zevra absorbed debt structured around the Acer acquisition, which requires disciplined capital allocation to service without squeezing growth capital.

5. The Mispricing Framework

Wall Street continues to misprice Zevra as a high-risk, cash-burning pre-revenue biotech.

In reality, Zevra is a commercial-stage rare disease provider with:

1. An FDA-approved, first-in-class asset with a captive patient base.

2. A $100M non-dilutive cash floor from its PRV sale.

3. Accelerating top-line commercial revenue.

This disconnect creates a classic asymmetric mispricing window: the downside is heavily protected by hard cash and approved commercial assets, while the upside remains wide open as MIPLYFFA adoption scales.
